                  For CPAP: Your specific single pressure setting: ___ CM/H20
                  For APAP: Your pressure range from ___ to ___ CM/H20
                  ✔ Your physician’s signature
                  ✔ Your physician’s contact information (including fax)
                  ✔ The patient’s full name
